Steps to obtain academic accommodations:
Student Accessibility and Accommodations (SAA) uses , a personalized accommodation software. Students and faculty can access the software using their Emmanuel credentials.
1. Students must self-disclose their disability/impairment by and completing the Self-Disclosure Form. This form can be accessed in via the "Accommodation" drop down menu in the left sidebar, which will reveal a link to the Self Disclosure Form.
2. After the Self-Disclosure Form is completed, students must upload appropriate documentation from a diagnosing health care provider into their student portal. This can be uploaded in via the "Accommodation" drop down menu, in the sub-category entitled "Documents." More information on what constitutes documentation from a qualified evaluator can be found within the documentation guidelines below.
3. Once both requirements are met, SAA will contact students via email to schedule an academic intake meeting. This is typically a one-on-one meeting with a member of SAA staff where documentation will be reviewed, reasonable accommodations will be determined and accommodation forms will be sent to faculty.
Please Note: Students may self-disclose at any point in their college career. However, for academic accommodations to be implemented in the current semester courses, intake meetings must be held prior to the last full week of classes.
Academic Accommodations are determined by:
- The barriers or limitations resulting from the interaction between the documented disability and the campus environment;
- The possible accommodations that might remove the barriers;
- Whether or not the student has access to the course, program, service, job, activity or facility without accommodations;
- Whether the possible accommodations would fundamentally alter the nature of or affect the essential elements of the course, program, service, or facility;
- Whether providing the accommodations would result in an undue burden for the College.
Academic accommodations are determined based upon individual need and professional recommendations.
Documentation Guidelines
Documentation must be œcurrent."
- For psychiatric/psychological, medical, sensory or other health diagnoses:¯completed within the last six months
- For specific learning disabilities, ADHD: within the last three years
Requests for academic accommodations must be relevant and applicable to the student current diagnosis. The documentation criteria are as follows:
- A letter or report written on official letterhead, signed by an evaluator who is qualified to diagnosis the student disability/impairment which includes:
- Evaluator license/certification, date of last appointment, diagnosed disability/impairment with DSM/ICD codes, list of test instruments in evaluation, any related history, functional limitations, and recommend supportive reasonable accommodations.
- Or, the ¯fully completed by the student diagnosing health care provider.
Please note: Documentation does not carry over from high school to college. A¯high school 504 plan and IEP are K-12 documents, though the accommodations contained within may be applicable in the collegiate setting. If submitting an IEP or 504 plan, please also include a recent letter, report, or the Medical Verification Form from the diagnosing provider in addition.
